Title: I can't change my clan, neither my name

i want to ask why i can't change my name at adrenalinex.co.uk/rename

everytime i try to change my clan or name it always show this page (http://i1190.photobucket.com/albums/z444/Rizqi_Fauzan/AX.jpg)

but i have entered the right password but the page still like that

well i already put my ingame password but still that page comes out

have you fix it yet? because my name is still not on FDR member list and i still cant change my clan
