actually that was the first thing that i thought about when i saw this, but yeah, that is what i meant by learning from mistakes.
they learned that the double metal ring wheels with amortizing rubber padding in between were not durable enough, and switched back to less comfortable, but durable solid metal wheels.

we don't mean it this way.
of course its better if it doesn't happen, but when it happens instead of ripping driver apart and putting all the blame on him the case should be investigated so it can be prevented next time.
it's very easy to say that the driver was just an idiot but that won't help as much as as a flaw found would.
that brings to the point im saying all the time, the driver might be an idiot, but saying "there is no mystery, totally his fault" before any investigation or matching info is just stupid and doesn't help anyone.
